We provide comprehensive and integrated mental health services for clients who have mental health problems and may have co-occurring problems such as homelessness, poverty, domestic violence, and other factors that prevent the best quality of life. Our Case Coordination services provide a holistic approach to address standard determinants of health (SDOH) and other barriers to aid our clients with receiving the best life possible for themselves. This service is only for clients who recieve therapy services.
